New Patient Registrations


New patients should attend the surgery to join our list. If you have a medical card please bring this with you, otherwise you will need to complete a GMS1 form at our reception or you can download a copy using this link https://assets.publishing.service.gov.uk/government/uploads/system/uploads/attachment_data/file/1017019/GMS1-family-doctor-services-registration-form.pdf


You will also need to complete our practice registration form, you can do this in the surgery  when you register at the practice.

Medical treatment will be available within 24 hours from the date of registration. Please contact reception for further information.

Prescription ordering -

You can order your medication by post or in person, you can also order online via Patient Access.  You need to register for this service and our reception staff will be happy to help you.

If you are over 70 years of age you can order your medication by telephone.

If you care for a vulnerable patient there is an alternative email address where you can request medication, please contact the surgery for this.


You can ring for test results between 12pm - 2pm and 5pm - 6pm Monday to Friday.

7.30am - 6.30pm Monday - Friday


Out of Hours - ring 1-1-1


We now have extra appointments available with the Practice Nurse or Phlebotomist between 7.30am to 8.00am
